Infiltrator. Pretending to be his creator, Hank Pym, Ultron was briefly named onto the Unity Division roster in Uncanny Avengers (3rd series) #9-10. Later on, "Mark Twelve" joined the ancillary roster of Avengers, Inc. #1.
Notes: 

Ultron-5 debuted in Avengers (1st series) #54 in the identity of the Crimson Cowl. Ultron-1 was not seen until flashbacks to his creation in #58. Ultron continued following a rough numerical sequence through the appearance of Ultron-16 in Avengers (3rd series) #19-22. There, he produced hundreds of versions of himself, and has largely ignored any numerical categorizing since.

Ultron successfully evolved to the point where he conquered the world following Avengers (4th series) #12.1 and the Age of Ultron event. Thanks to the use of time travel, the events of #12.1 were rewritten and Ultron was destroyed by an Anti-Ultron Virus. The Ultron currently appearing is actually derived from an older version circa Ultron-8, who traveled through time from the past to the present in Avengers: Rage of Ultron. However, by connecting to the modern data cloud, this Ultron-8 apparently upgraded his memory to include all past versions he skipped over.

From Avengers: Rage of Ultron, Ultron and Hank Pym fused in an amalgamation of organic matter and robotic technology. "Ultron Pym" has been inconsistent about the true nature of its identity. It claimed to be merely Ultron, having killed Hank Pym and assimilated a copy of his personality and memories to use when convenient. It also claimed to be Hank Pym, driven to madness and voluntarily working with Ultron as a co-dependent mind.

Following Ant-Man (3rd series) #1-4, Hank Pym was restored as a living being. However, he was under the influence of the Ultron Imperative, using robot ants to subconsciously house Ultron's mind for his return. Pym managed to subvert the process by implanting the benevolent Ultron Mark Twelve into one of the ants, which in turn linked with the body of David Cannon, aka Whirlwind. As an amnesiac and distinct personality, the body of Whirlwind initially chose the name "Victor Shade" after Vision's old alias, before eventually regaining his memories and rewriting over all Ultron versions to become the definitive version, Mark Twelve. This all took place in Avengers, Inc. #1-5.
