1) The original Cobalt Man armor was directly based off of Iron Man’s armor at the time. The cobalt blue armor was a full body suit containing cybernetically controlled micro-circuitry and a flexible exoskeleton. The face plate had rectangular openings for his eyes and mouth. His radioactive Cobalt-60 power pack was operating on his chest plate, producing a glowing reaction. [X-Men (1st series) #31]
2) After becoming a radioactive super-being, Ralph modified his Cobalt Man armor to be larger and channel his self-produced energy. The new suit resembled cobalt blue chainmail, with larger metal armor on top. He had an atomic symbol represented on his chest. Cobalt Man’s helmet almost resembled a football helmet, with an exposed face but a large bar across his nose. [Incredible Hulk (2nd series) #173]
3) The Cobalt Man Mark III armor removed the open-faced look of the previous suit, going for a fully contained helmet. If reports are true, this version of Cobalt Man may have been an insubstantial energy cloud without the armor, explaining why it covered him so completely. The cobalt blue armor had a power pack on the chest like the original version, with the second version’s atomic symbol now on each shoulder. Although this armor was first worn by Iron Man while posing as Cobalt Man, the real Cobalt Man apparently adopted it as well some time later. Ralph Roberts' version was substantially larger, though. [Avengers / Thunderbolts #1 (Stark); Deadpool & the Mercs for Money (2nd series) #3 (Roberts)]
